1095-A Federal Marketplace Health Insurance Statement

Sent to individuals who purchased health insurance through the Marketplace (Healthcare.gov). This form shows the months you were covered, your premium amounts, and any advance premium tax credits received, and is required to complete the Premium Tax Credit section of your tax return.
